Rockford Memorial Hospital
Financial Assistance and Discount Programs
Rockford Health System provides health care
regardless of the patient’s ability to pay for such care. These
guidelines are intended to provide a framework to offer assistance
to patients and families with legitimate financial hardship, who are
unable to pay for all or a portion of their medical care.
COVERED SERVICES
Covered services are those which are determined medically necessary
by a physician.
EXCLUDED SERVICES
A. General dental
B. Cosmetic
C. Guest trays
D. Private room (unless medically necessary or no other room
available.)
Our
Financial Assistance Programs are based on whether you have
insurance coverage of any kind or are uninsured.
An
uninsured patient
is an Illinois resident who is
not covered under any third party coverage of any kind, including
high deductible plans, workers’ compensation, accident liability
insurance, or other third party liability.
This follows the Illinois Hospital Uninsured Patient Discount
Act.
