Cardiac Risk Assessment
Physicians at the Heart and Vascular
Center at Rockford Health System are
pleased to offer a comprehensive
assessment to help you find out your
risk for heart disease. Heart
disease is the number one killer in
America; however, many people
ignore the warning signs.
Learn what the warnings signs are
and what you can do to prevent them
or keep them in check.
The cardiovascular risk assessment
is the first step toward living a
more heart-healthy life. You
will receive a one-on-one private
consultation with a nurse
practitioner in collaboration with a
cardiologist from the Heart and
Vascular Center at Rockford Health
System.
The assessment will include:
- A screening for risk factors
- Cholesterol panel
- Blood pressure
- Blood sugar
- EKG
- A weight check with body mass
index and hip/waist ratio
- Peripheral/vascular evaluation
- Counseling on diet, exercise and
smoking cessation, as needed
The cost is $25. The cardiac
risk assessment is available by
appointment only at Rockford Health
