Diabetes and Nutrition Services

Diabetes is the leading cause of new blindness, kidney failure, nerve disease, and cardiovascular disease; and is costing our country 218 billion dollars in direct medical costs. Rockford Health Physicians Diabetes and Nutrition Services offer several educational opportunities from Certified Diabetes Educators, RN and a Registered Dietitian.   

DIABETES SELF MANAGEMENT EDUCATION CLASSESS AND CONSULTS

If you or someone you know has Diabetes; Rockford Health Physicians offers Diabetes Self Management Education through their Diabetes and Nutrition Services Program accredited by the American Association of Diabetes Educators.  Diabetes self-management education includes gaining the knowledge, skills and confidence necessary for taking control of your diabetes. This process incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. These classes will support informed decision-making, self-care behavior changes, and problem-solving. We will work with your entire health care team to improve your health status, and quality of life. 

MEDICAL NUTRITION THERAPY

Healthy eating is a key component of good health. Our Registered Dietitian offers education and counseling geared towards helping you to adapting healthier lifestyle, prevent or manage chronic disease, and achieve or maintain a health weight. Learn the skills needed to incorporate healthy habits into daily living.

INSULIN PUMP THERAPY AND CONTINOUS GLUCOSE MONITORING

The diabetes educators of Diabetes and Nutrition Services are all certified in insulin pump training and continuous glucose sensor training.  A 3-5 day blinded continuous glucose study may be required by some insurance companies before a home used continuous sensor is approved or needed to aid in better management with insulin, or even to make a diagnosis.  We also have the ability to do these and coordinate with the Endocrinologists and the endocrine nurse practitioner to do the follow-up interpretation.  Ongoing insulin pump management is normally done in conjunction with the Endocrinologists, seeing patients as a shared medical visit.  Patients are welcome from other providers for a consult with the diabetes educators to obtain a refresher and to troubleshoot insulin pump or multiple daily injections self-management.
